• “Alliance partner” shall mean Phasorz Technology Pvt Ltd, who has entered into an alliance agreement with ICICI Bank for purpose of providing the Offer.
• “Customer/s” for the purpose of this Offer shall mean a person who is using ICICI bank Netbanking services or holds a valid ICICI Bank VISA/MASTER Debit card/Prepaid card or holds a valid VISA/MASTER Credit Card issued by ICICI Bank and who has received communication from ICICI Bank with respect to the Offer.
• “ICICI Bank Net Banking User” shall mean a person who holds an ICICI Bank savings account, loan / facility account(s), depository account(s) and/or any other type of account(s), so maintained with ICICI Bank or its Affiliate which are eligible account(s) for purposes of ICICI Bank Net Banking Service(s) and who has received communication from ICICI Bank with respect to the Offer.
• “Cardholder” shall mean a customer to whom the Facility has been granted by virtue of such customer holding a Card.

1. The Service Provider reserves absolute right at any time to add, alter, withdraw, modify or change or vary any or all the terms and conditions of the offer at its sole discretion and the same shall be binding on the Alliance Partner at all times.
2. The Alliance Partner shall not make any warranties on services, discounts, service levels, etc other than those covered herein the Customer Agreement or as amended from time to time.
3. The Service Provider agrees to provide doctor consultation services in accordance with the Fair Usage Policy (FAU) on the date of purchase or as on renewal date. Currently, the FAU is defined as a total of 6 Online Consultations per person for a subscription period of 12 months.
4. All in-clinic OPD consultations are restricted to network doctors only, and appointments shall be sought via Service Provider’s app or customer care
5. Currently, the subscription is made available at an introductory price inclusive of GST, and price for all variants are applicable for only for 12 months from the date of purchase of each subscription.
6. All prices are reviewable after one year, however, the price offered for two and three year plans are reviewable only after 2 & 3 years from date of purchase.
7. All discounts offered may be reviewable annually, and will be communicated to the subscriber on their renewal date.
8. Additional in-app discounts are available for delivery of medicines and on booking lab tests, however, these are not contractual obligations and are bound to change.
9. In the year one, all follow up online consultation within 3 days from the date of preceding online consultation will not be counted towards FAU. However, any online consultation after 3 days from the preceding online consultation will be considered as a new consultation, and for all practical purposes, a day is defined in a conventional way.
10. No cancellation will be affected, or refund made (fully or partially) after 10 days from the date of subscription irrespective of the duration of the contract. However, should the customer have availed any one consultation, no refund will be made even in the first 10 days unless deficiency of service is proven.
11. No compensation whatsoever is payable to the Customer for any deficiency or non-availability of services other than the refund of the value of the consult. Whilst the Service Provider’s endeavor is to provide quick and efficient services in the current location of the subscriber or the member of the subscription, it may not always be able to guarantee availability of such services. However, if non-availability of doctors is experienced by the customer or their covered members for more than two consecutive consultation requests per member per month, the subscription value will be refunded on a prorata basis at the request of the customer. No other compensation will be payable.
12. Any premium services introduced by the Service Provider during the subscription period do not automatically become part of the subscription, however, the Service Provider at its sole discretion may choose to do so for any one or group of customers.
13. Any abuse of the service may lead to complete termination of the subscription without any refund.
14. This subscription is available only to individuals who have chosen to buy this for themselves or their family, and is not available to any individual or organization that may resell the subscription in whole or part or add members for any fee or consideration. This will also be considered as misuse of service. However, genuine cases of emergency or subscription purchased on behalf of a done are exempted from this, and the Service Provider has the final say on what constitutes misuse.
15. Any consultation taken above the FAU will be subject to review by the Service Provider, and the Service Provider at its discretion may charge differently
16. Refunds or Discounts, if any, will only be made to the Customer wallet except where subscription is also terminated.
17. All IPD & OPD admissions should be sought via the app or the call centre of the Service Provider, and the discounts will be available only for network hospitals, and when paid in full by the individual, and not on any negotiated contracts
18. The list of network providers will be updated from time to time, and will be available for access by the customer

a) Terms prescribed by for ICICI Bank Netbanking Users

• ICICI Bank does not guarantee and make any representation about the usefulness, worthiness and/or character of the discount / benefit or of the Products/Services under the Offer provided by the Alliance Partner.
• Any tax or other liabilities or charges payable to the government or any other statutory authority/body or any participating establishment, which may arise or accrue to the ICICI Bank Net Banking User/s/ due to provision of the Offer, shall be to the sole account of the ICICI Bank Net Banking User/s. Tax deducted at source, if any, on the monetary value of the Offer shall be payable by the ICICI Bank Net Banking User/s.

The same shall be addressed by Alliance Partner, only up to a period of 15 days after the date of transaction/purchase.
• The existence of a dispute, if any, regarding the Offer shall not constitute a claim against ICICI Bank and shall be addressed directly by the Alliance Partner.
• The Offer is not available wherever prohibited and / or on products / services for which such offers cannot be made available for any reason whatsoever. 
• ICICI Bank/Alliance Partner reserve the right to modify/ change all or any of the terms applicable to the Offer without assigning any reasons or without any prior intimation whatsoever. ICICI Bank also reserves the right to discontinue the Offer without assigning any reasons or without any prior intimation whatsoever.
• ICICI Bank reserves the right to disqualify the Alliance Partner/s or ICICI Bank Net Banking User /s from the benefits of the Program if any fraudulent activity is identified as being carried out for the purpose of availing the benefits under the Program or otherwise by use of ICICI Bank Net Banking.
• The Offer is sponsored by the Alliance Partner and the ICICI Bank Net Banking User shall be bound by the terms and conditions stipulated by the Alliance Partner in this regard.
• ICICI Bank shall not be held liable for any delay or loss that may be caused in delivery of the Products and Services or the assured gifts / prizes.
• The Program/Offer is not available wherever prohibited and / or on merchandise / Products / Services for which such programs cannot be offered for any reason whatsoever.
• If the ICICI Bank Net Banking User ceases to be an ICICI Bank Net Banking User at any time during the currency of the Offer Period, all the benefits under the Offer shall lapse and shall not be available to the ICICI Bank Net Banking User.
