- The documents required for making a Major Medical Illness & Procedures claim are as follows:
- Doctor's Certificate confirming Insured person's name, date of occurrence and medical details of the illness / surgery defined and covered under the policy
- Duly completed claim forms
- Attested copy of PM (Post-mortem) report
- Original/ attested copies of Discharge Certificate/ Card from the hospital/ Doctor
- Original/ attested copies of investigation test reports, indoor case papers
- Any other supporting documents as may be required by the Company
Documents required for Accidental Death and Permanent Total Disability due to accident
- The documents required for making claims for an Accidental Death and Permanent Total Disability due to accident are as follows:
- Duly completed claim form
- Doctor's Report
- First Information Report and Final Police report, wherever necessary
- Death certificate, wherever applicable
- Investigation Reports like Laboratory test, X-rays and reports essential of confirmation of the Injury
- Disability certificate from a government certified Doctor or government hospital confirming the extent and nature of disability
- Post mortem report, if applicable
- Any other supporting documents as may be required by the Company
