How to apply

Step 1: Please click here to download the application form.

Step 2: You will have to submit the following documents along with the application form at the nearest ICICI Bank branch:

• Photocopy of the Supplementary Card applicant’s Permanent Account Number (PAN) card,self-attested by the Supplementary Card applicant.If   Supplementary cardholder does not have a PAN card then,Form 60 and ID proof with customer acknowledgement of PAN Application to be collected.
•  Photocopy of the Supplementary Card applicant’s Address Proof , self-attested by the Supplementary Card applicant.
•  Photocopy of the Supplementary Card applicant’s ID Proof , self-attested by the Supplementary Card applicant. (Not required if PAN card is provided)

Keep the original copy of documents handy for verification.
