How to claim the insurance cover?

  • In case of any claim, the claimant should inform ICICI Lombard call centre on 1800 2666, within 15 days of loss or damage.
  • ICICI Lombard will inform the claimant about the documents required for processing the claim.
  • The claimant should send the required documents to ICICI Lombard.
  • The Customer Service department of ICICI Lombard will decide on the admissibility of any claim as per the Terms & Conditions of the policy, after receiving the documents and investigation report, if applicable.
  • On admissibility, ICICI Lombard will make the payment to the beneficiary. The payment shall be made within 15 working days of receipt of all complete documents, in case an investigation is not required.

List of documents required to claim insurance

Air Accident (death only)

  • Personal Accident (PA) Claim form duly filled in and signed by the Nominee – Original.
  • Card statement prior to date of incident along with the transaction showing the purchase of ticket for the travel.
  • Copy of Death Certificate - Notarised/Attested by Gazetted officer. 
  • Copy of FIR - Notarised/Attested by Gazetted officer. 
  • Copy of Post mortem report - Notarised/Attested by Gazetted officer. 
  • Copy of Panchnama (Spot and/or Inquest) -Notarised/Attested by Gazetted officer.
  • Treatment papers, if hospitalised.
  • If claim amount > Rs 1 lakh, AML documents - Notarised/Attested by Gazetted Officer (PAN Card Copy, Address proof, 2 Passport size colour photographs of claimant).
  • Indemnity cum declaration bond on Rs 50/100 stamp paper (legal heir certificate) – Original.
  • Consent letter from other legal heirs on Rs 50/100 stamp paper (No Objection Certificate by other legal heirs) – Original.
  • NEFT mandate form duly filled in by the claimant and verified by bank with cancelled cheque of the nominee - Original (for direct fund transfer).

Personal Accident (other than air accident)

  • Personal Accident (PA) Claim form duly filled in and signed by the Nominee – Original.
  • Copy of Death Certificate - Notarised/Attested by Gazetted officer. 
  • Copy of FIR - Notarised/Attested by Gazetted officer. 
  • Copy of Post mortem report - Notarised/Attested by Gazetted officer. 
  • Copy of Panchnama (Spot and/or Inquest) -Notarised/Attested by Gazetted officer.
  • Treatment papers, if hospitalised.

Purchase Protection

  • Claim form duly filled and signed by the customer.
  • Card copy.
  • Copy of FIR - Notarised/Attested by Gazetted officer. 
  • Card Statement showing Purchase details of the item lost.
  • Incident letter by the customer.
  • Purchase bill for the item lost: Original.
  • If claim amount > Rs 1 lakh, AML documents - Notarised/Attested by Gazetted Officer (PAN Card Copy, Address proof, 2 Passport size colour photographs of claimant).
  • NEFT mandate form duly filled in by the claimant and verified by bank - Original (for direct fund transfer).
  • Cancelled cheque - Original (for direct fund transfer).
