Understand Critical Guard
|
- What is Critical Guard?
Critical Care protects you or your spouse against loss
of income on diagnosis of any of the 9 major medical
illnesses and procedures. The first of its kind, it
offers a lump sum benefit on diagnosis of Cancer, Bypass
Surgery, Heart Attack, Kidney Failure, Major Organ Transplant,
Stroke, Paralysis, Heart Valve Replacement Surgery or
Multiple Sclerosis. Critical Care Insurance also provides
cover against accidental death and permanent total disablement
(PTD).
- Choice of Coverage
Sum Insured Rs. 3, 6 or 12 Lakhs
Critical Care offers you a choice of coverage
on both the sum insured and the tenure of the policy.
You can choose the sum insured of Rs 3, 6 or 12 Lakhs
over a period of 1, 3 or 5 years. The premium would
be calculated accordingly: The cover would include:
- Major Medical illness and procedures
- Accidental Death
- Permanent Total Disablement (PTD) on account of
Accident leading to inability to remain gainfully
employed

Major Medical Illnesses and Procedures Cover
The Critical Care Insurance shall cover, subsequent
to 90 days from the policy start date, the following
major medical illnesses and procedures:
- Cancer
- Bypass Surgery
- Heart Attack
- Kidney Failure (End Stage Renal Failure)
- Major Organ Transplant
- Stroke
- Paralysis
- Heart Valve Replacement Surgery
- Multiple Sclerosis

On diagnosis of any of the above illnesses, the insured
is entitled to the lumpsum benefit of the entire Sum
Insured opted for.
Accidental Death Cover
In case of death of the insured due to an accident
within the policy period, the nominee (mentioned in
the policy) is compensated with the Sum Insured (opted
at the time of policy issuance).
Permanent Total Disablement (PTD) Cover
Critical Care insures against the permanent and total
loss of limbs, sight etc., due to an accident. The compensation
(Sum insured opted for) is given as a lump sum benefit.
It is to be noted that the compensation is payable only
if the disablement results in inability to remain gainfully
employed.

- What documents do I need?
Critical Illness Claim
The documents required for making a Critical Illnesses Claim are as follows:
Doctor's Certificate confirming the following:
- Insured person's name
- Name, date of occurrence and medical details of the illness/surgery
- Confirmation of the absence of any pre-existing Illness or Injury within the first 3 months of policy start date
- Duly completed claim forms
- Original Discharge Certificate/ Card from the hospital/ Doctor
- Original investigation test reports, indoor case papers
Personal Accident Claim
The documents required for making a Personal Accident claim are as follows:
- Duly completed claim form
- Doctor’s Report
- First Information Report and Final Police report, wherever necessary
- Death certificate, wherever applicable
- Investigation Reports like Laboratory test, X-rays and reports essential of confirmation of the Injury
- Disability certificate from a government certified Doctor or government hospital confirming the extent and nature of disability
- Post mortem report, if applicable
- Any other supporting documents as may be required by the Company
- How do I make a Claim?
- In order to make a claim, make an immediate notice
in writing to the Company
Critical Illness Claim: Intimate ICICI Lombard within
30 days from the date of diagnosis of the concerned
illness/surgical procedure
Personal Accident Claim: Intimate ICICI Lombard within
7 days from the date of occurrence of the accident/
injury
- Submit all reasonable documents - information, assistance
and proofs in connection with any claim within 30
days from the occurrence of the event.
