SEAFARER's submitting application for account opening at ICICI Bank Branch in India or Overseas  
     
Photocopy of the relevant pages of your current passport where your name, address, date of birth, date & place of issue, expiry date, photograph & signature appear. The photocopy must include the first 4 pages of passport & pages indicating last date of arrival in India. The signature on your passport should be clearly visible and should exactly match the signature on your Registration Form.  
   
C 1 Visa OR CDC (Continuous Discharge Certificate). The certificate must indicate last date of arrival in India or declaration by ICICI employee (during the branch visit) confirming that you are sailing for the first time  
   
Declaration letter (as per specimen on last page of the Deposit Form)
 
   
Employment Proof

a) IF YOU ARE A PERMANENT EMPLOYEE YOU NEED TO PROVIDE:
Initial work contract AND
Last wage slip issued within last 6 months

b) IF YOU ARE A CONTRACT EMPLOYEE YOU NEED TO PROVIDE ANY ONE OF THE FOLLOWING WHICH IS APPLICABLE:

If contract is valid as on date, you need to provide

- Copy of Current contract letter
or
- Letter from Shipping Company/Agent/Liaison Office confirming that he is presently employed with the company. The letter should not be more then 3 months old.

If contract has expired within last 9 months, you need to provide

- Copy of last Contract Letter
or
- CDC Copy
- Visa Copy along with Passport having immigration seal within 6 months

If contract has expired prior to 9 months, you need to provide

- Current Work Contract Letter
or
- Letter from Local Agent confirming next date of joining vessel

Attestation on the photocopies of all the above documents to be done by any one of the following - Banker / Financial Institution / Credit Unions/ Indian Embassy / Notary (or equivalent of notary as per applicable law).

Any one of the following 3 documents

- Copy of any letter received from your existing banker in India / Abroad (dated within last 6 months)
 - The communication should be on the Letterhead of the bank, stamped and signed.
 - Electronically generated statements not requiring signature or documents with printed signatures will not be accepted.

- An original cheque issued by you and paid by your banker outside India. The cheque should have cleared in the last 6 months. For banks which no longer return paid cheques, the customer can take a print of the same from Internet banking login and get it stamped from the same bank.

- Copy of the latest statement of bank account or relevant sheets of passbook of your account in India / Abroad (atleast one month statement with minimum 2 customer induced transactions)
